Medication Review/Annual Care Plan

With the Standard Medication Management Assessment (SMMA) and Comprehensive Annual Care Plan (CACP), Brintnell Pharmacy provides greater access to health care. Our pharmacists can help with medication review, assess your needs, provide care plans and follow-ups, and collaborate with your other health care providers.

STANDARD MEDICATION MANAGEMENT ASSESSMENT & COMPREHENSIVE ANNUAL CARE PLAN (SMMA/CACP)

As pharmacists, we work closely with patients and have a unique perspective of patients, their medical needs, and medications. From our position, we are uniquely qualified to take the leading role in actively helping you get to know and understand your medications. With the Standard Medication Management Assessment (SMMA) and Comprehensive Annual Care Plan (CACP), we are proudly able to provide even more people with access to quality health care.  

If you have chronic medical conditions for which you need to take medications, our pharmacists at Brintnell Pharmacy and Travel Clinic can help you to better understand your medications and medical conditions. With easier and faster access to pharmacists, it’s easier than ever before to ensure that you can take your health into your own hands and keep your chronic medical conditions under control. 

In collaboration with you and your other health care providers, our pharmacists will assess your medication needs and provide care plans and follow-ups. 

SMMA/CACP CONSULTATIONS

Residents of Alberta with a valid Alberta Health Number under the Alberta Health Care Insurance Plan (AHCIP), are eligible for the provincially funded Standard Medication Management Assessment (SMMA) or Comprehensive Annual Care Plan (CACP). Visitors to Canada, non-residents of Alberta, or those without active Alberta Health Number under the AHCIP, should expect to pay a $50 fee for either the SMMA or CACP consultation.  

Brintnell Pharmacy and Travel Clinic is proudly led by owner-pharmacist Oluwaseun Agunbiade (aka Olu), a proud member of several associations, including the Alberta Pharmacists Association, and Canadian Pharmacists Association, the International Society of Travel Medicine, the College of Psychiatric and Neurologic Pharmacists, and has additional prescribing authorization.
